Palm Healthcare Foundation announced that it has awarded 35 grants totaling $1,919,513 to local non-profits. Together, the funding will support the increased health and wellbeing of Palm Beach County residents.  All of the grant dollars were awarded in fiscal year 2016.

“For the past 15 years, Palm Healthcare Foundation has been committed to solving the county’s critical healthcare issues through creative, community-wide collaborations,” said Andrea Bradley, J.D., President and CEO. “We focus our resources on community-based care, as we work with targeted neighborhoods to address the underlying causes of health issues. There is much work to be done, and with the support of our many partners, we can tackle these issues directly and thoroughly.”

Palm Healthcare Foundation’s grants to area health organizations in FY2016 include:

211 Palm Beach Treasure Coast, Inc.($10,000)

Funds provided create 211 customized community resource directories utilized for the foundation’s Healthier Together targeted communities.

American Cancer Society, Inc.($81,513)

Cancer Prevention, Education and Early Detection in Palm Beach County

The funding expands resources to educate patients on cancer prevention and early detection with a focus on nutrition, physical activity and tobacco cessation.

Blue Ridge Institute Development Fund, Inc.  ($2,500)

Cancer Alliance of Help and Hope, Inc. ($2,500)

Patient Financial Support Program

Funding supports Palm Beach County residents undergoing cancer treatments with living expenses, health insurance, car payments, food and other basic necessities.

Caridad Center, Inc. ($30,000)

Equipment and Supplies for Vision Center – Donor Advised Fund

The BJ and Paula Harris Donor Advised Fund at Palm Healthcare Foundation will provide equipment and supplies for a four-room vision clinic at the Caridad Center, the largest free clinic in Florida.

Racial Equity Institute

Through Healthier Together, there is an increasing awareness that racism, discrimination and feelings of isolation contribute to health disparities in Palm Beach County and impacts collaborative work. Funding provides an opportunity for Healthier Together community members to attend a two-day Racial Equity Institute workshop.

Caridad Center, Inc. ($35,000)

Prevention, Education and Treatment Program

Funding supports the Prevention, Education and Treatment Program for high-risk diabetic Haitian and Hispanic patients to learn healthy lifestyles, exercise, nutrition and chronic disease prevention and management.

Clinics Can Help ($40,000)

Support for Capital Campaign – Donor Advised Fund

The BJ and Paula Harris Donor Advised Fund at Palm Healthcare Foundation provides partial funding for a permanent site for this organization. Clinics Can Help donates surplus medical equipment and supplies to Palm Beach County children and adults in need.

Community Child Care Center of Delray Beach, Foundation, Inc. (Achievement Centers for Children and Families)  ($200,000)

Healthier Delray Beach – Behavioral Health Education, Awareness and Access Initiative

This Healthier Together grant supports programmatic strategies to address behavioral health issues. Healthier Delray Beach is committed to addressing the stigma associated with behavioral health and to training residents and professionals on evidence-based practices for children and families who live in Delray Beach.

Community Foundation for Palm Beach and Martin Counties, Inc. ($10,000)

Great Give 2016

Funding provides matching funds to support local nonprofit organizations through this collaborative county-wide campaign.

El Sol Jupiter’s Neighborhood Resource Center (El Sol) ($25,000)

Sunshine Community Garden

This grant supports the Sunshine Community Garden that offers growing plots for individuals and families, healthy foods, a place for moderate physical activity and excess food that supplements El Sol’s lunch program and food pantry.

Florida Atlantic University, College of Business ($50,000)

Development, implementation and management of an Elder-Care Services Learning Community in Palm Beach County

Funding supports the roll-out of a learning community comprised of elder-care service providers in Palm Beach County to improve communications and coordination of care.

Florida Atlantic University Simulation Center ($50,000)

Enhancement of  a Simulation Training Program that trains family caregivers and home health aides

Funding supports the INTERACT program that trains family caregivers and home health aides to learn skills, identify appropriate and necessary interventions and report clinical changes to other healthcare providers.

Fred and Gladys Alpert Jewish Family & Children’s Services of Palm Beach ($80,000)

Mental Health First Aid: An Integrated Community Model

Funding supports the creation and coordination of an integrated community model to introduce the international, evidence-based Mental Health First Aid Program throughout Palm Beach County. The program teaches participants how to recognize and respond to risk factors and warning signs of mental health problems.

Diabetes Coalition of Palm Beach County-Diabetes Prevention and Management Program

Funding supports the Diabetes Coalition of Palm Beach County with a mission to prevent diabetes and improve the lives of people living with the disease, and those who are touched by it, through education, awareness, quality services, management and advocacy.

George Snow Scholarship Fund, Inc.  ($200,000)

Palm Healthcare Foundation Nursing Scholarship Program

Funding supports undergraduate and graduate nursing student scholarships in Palm Beach County. The George Snow Scholarship Fund will manage and monitor the scholarship awards.

Hospice of Palm Beach County Foundation, Inc.  ($25,000)

The William E. Flaherty Donor Advised Fund provides general operating support for end-of-life care.

Housing Partnership, Inc. ($26,000)

Funding supports a facilitated health dialogue in Lake Worth & the Glades communities named Health Dialogue to Action (HDTA)

Healthier Together Leadership Conference in 2016

Funding provided a one-day Collective Impact conference to strengthen Healthier Together communities’ knowledge and skills for effective resident engagement, goal-setting and concrete action steps

This organization serves as the fiscal agent for the Healthier Together collaboration between the cities of Northern West Palm Beach and Riviera Beach with a goal to improve the overall behavioral health of their residents.  Known as Healthier Neighbors, this Healthier Together initiative will work to find solutions to behavioral health issues in its neighborhoods.

Jupiter Medical Center Foundation ($200,000)

This organization serves as the fiscal agent for the Healthier Jupiter initiative. This Healthier Together initiative is addressing health and quality of life issues starting with diabetes prevention and management.

MyClinic ($25,000)

Capital Campaign- Donor Advised Fund

The BJ and Paula Harris Fund at Palm Healthcare Foundation supports a capital campaign to build a permanent building on the site of this not- for-profit, free clinic that provides primary medical care and urgent care dental referrals to low income, uninsured residents of Palm Beach County.

Northwest Community Consortium, Inc. ($75,000)

Health & Wellness Model for Faith Based Communities — Phase II

Funding supports the implementation of a program to increase access to healthcare, provide health education and literacy and provide Mental Health First Aid Training in the Northwest Community of West Palm Beach in an effort to strengthen the health of the residents and revitalize the neighborhood.

Palm Beach Atlantic University, Inc. ($22,500)

Online Healthcare Orientation (HCStudentsFL.org)

The School of Nursing at Palm Beach Atlantic University will take over responsibility, maintenance and development of this student orientation website that provides a standardized orientation for students completing clinical rotations at Palm Beach County healthcare facilities.

Palm Beach Atlantic University (PBAU) ($125,000)

Volunteer Nursing Corps Implementation

This grant supports the establishment of the Volunteer Nurse Corps, administered by Palm Beach Atlantic University’s School of Nursing. The corps is a group of nursing faculty, nursing students and nurses working in Palm Beach County, who focus on care for vulnerable populations by providing health information, education and a helping hand to individuals who are leaving acute care settings to live in their homes.

Palm Beach Cancer Institute Foundation/Sari Asher Center for Integrative Care ($5,000)

General Support – Donor Advised Fund

Funds from the Gary Levin Patient Scholarship Fund held at Palm Healthcare Foundation support cancer patients who are unable to pay for therapeutic treatments and therapies that help reduce symptoms and improve the quality of daily living.

Nutrition Driven Mobile Food Pantry

Funding supports a mobile food pantry that distributes fresh and healthy food with an education component to residents in low-income/limited access areas.

Palm Beach County Youth for Christ (PBCYFC) $50,000)

Journey to Holistic Health and Wellness: Body, Mind, Spirit

Funding provides programs for parents and their children that focus on physical and emotional health and wellness and parenting skills. Funding provides “Mom Manuals” that have medical and resource information for both parents and children.

Pathways to Prosperity, Inc. ($200,000)

Healthier Boynton Beach – Family Caregiver Support and Education

This organization serves as the fiscal agent for the Healthier Boynton Beach initiative. This Healthier Together initiative that addresses family caregiving challenges is seeking ways to better support family caregivers and their loved ones who have chronic health conditions and need assistance.

Welcome Home Capital Campaign/General Operating Support – Donor Advised Fund

The Gronewaldt Fund B partially supports the expansion of the existing facility with 20 additional rooms to offer supportive home-like care to lessen the burden for families whose children are receiving treatment in Palm Beach County for serious medical conditions.

South Florida Science Center & Aquarium, Inc. $15,000

Our Body: The Universe Within

The foundation is partnering with The South Florida Science Center and Aquarium to present this exhibit that aligns with the foundation’s philosophy that community engagement and education can lead to our residents’ ability to live healthier lifestyles.

The Glades Initiative, Inc. ($3,000)

Cooking Matters Nutritional Education in Haitian Creole

Funding enables the organization to translate the Cooking Matters Nutrition Education Program materials into Haitian Creole. The course teaches food-insecure residents how to prepare healthy meals on a low budget.

Truth Point Church ($10,000)

Homeless Resources Supplies (“Love Bags”)

Funding provides bags full of nutritional items along with health and hygiene products to Palm Beach County’s homeless population.

Wellington Cares, Inc.( $2,500)

Neighbors Helping Neighbors

Funding supports volunteer training sessions, materials, background checks and gift cards to enable the program to reach its goal of allowing seniors in Wellington to stay in their homes with the support of volunteers who offer free, non-medical services to the 65+community.

Young People In Recovery (YPR) ($2,000)

Palm Beach County Technical Assistance Program

Funding helps Palm Beach County chapters expand their work to conduct free housing, education and employment workshops to young people in recovery.
